Can you have breast reduction and go from a DD to an A cup?

I don’t know why, but I’m sort of obsessed with having small boobs. Maybe not A cups, but small perky Bs would be great. Is it recommended, or even possible to go that small if you have DDs? I’m only 23 years old if that matters.

Plastic surgeons generally do not judge breast size reductions by cup size, but rather by how many grams of tissue we expect to remove.  That said, going from DD to A may be feasible, but it depends somewhat on your chest width and body shape.  You need to be left with enough volume to maintain a breast mound.  Otherwise, you will be left very flat without any projection ("pancake"-like).  You also need to have the blood supply maintained to your nipple/areolas, which likely will be repositioned during the reduction.  Oftentimes, patients who think they are DD's are not as large as they think but rather have a lot of sagging which makes them think they are that large.  I suggest you consult with a board certified plastic surgeon to discuss your goals and your options.
