Is there a such thing as male areola reduction surgery?
My friends literally call me pepperoni nips every time I take my shirt off, which I try to avoid at all costs (can you blame me). I’ve learned to deal with it even though deep down I am pretty self-conscious. The worst part is that I’m in pretty good shape, and if it weren’t for my big areolas I wouldn’t mind having my shirt off in public. My girlfriend tells me they look fine, but I think she’s just being nice. Is it possible for me to have a procedure to help reduce the size of my areolas? I would think there has to be since it looks like a lot of plastic surgeons offer this for women.
Surgery to reduce the diameter of the areola is indeed possible, although in my experience it’s quite rare to see large areolas on a male - independent of gynecomastia/puffy nipples, at least.
The average size of male areolas is 20 - 25 mm. Without a consultation it’s difficult for me to say if your issue is really oversized areolas or if it’s prominent nipples due to excess tissue beneath the nipple.
If the issue is in fact gynecomastia I’d recommend you consider liposuction to remove the excess breast tissue. Male breast reduction can be accomplished with laser-assisted liposuction. Typically a visible reduction in nipple size is accompanied with fatty tissue removal. Also, liposuction will not result in a circumareolar scar that would result if we simply reduced the size of the areolas surgically.
Either way, there are definitely options available to you. I’d recommend consulting a board-certified plastic surgeon to determine the best approach for your specific case.

Oftentimes there is excess chest tissue underneath which is stretching out the areola. Only a physical exam with an experienced plastic surgeon can determine this. If you do have excess tissue (ie gynecomastia), then usually the areolas will shrink substantially when the excess tissue is removed. This also includes "puffy nipples", which is when the areolas protrude further from the surrounding skin (a dome-shaped areola). After appropriate surgery, those areolas will lie flat and flush with the surrounding skin.
It is extremely rare to have an absolutely flat, masculine chest simply with very wide areolas. In those rare circumstances, then perhaps an areolar reduction procedure could be beneficial.
